It depends on the ticket you have or you're intending to buy. If it's an
ticket valid on Olimpic only (Consolidators sell these tickets) you
can't use it for anything than flying Non-Stop to Athens and back.
If not its depends on your itenary.
Is it NYC-Athens-Bologna-Madrid-Bologna-NYC or NYC-Athens-Bologna-NYC.
In the first case buy an open-Jaw tickets (eg. Delta USD 1309) NYC-Athens/Madrid-NYC. Combine this with a full-fare Economy Athens-Bologna-Madrid USD ~675 or an train/ferry-ticket (much cheaper and more to see on the way) for this route.
In the second case look for an Alitalia ticket permittimg a stopover and fly USA-Milano-Athens-Rome-Bologna (stopover) -Milano-USA or Athens-Milano-USA with a stopover and sidetrip to Bologna.
