In my experience, mainly in Italy, it's best to opt out of breakfast at the hotel if at all possible. If breakfast is a separate part of the cost of the room, it's overpriced compared to the stand-up breakfast like the locals have at the bar next door. My experience dates back some time, but I've found that in hotels you can't count on a choice of pastries, and a cappuccino isn't proper, with foam. I know some hotels have tried to improve their offerings. If you're staying in a pensione you're generally expected to have breakfast there.
