Regarding Great Britain--free medical care is available for non-citizens
only on an emergency basis. I once had to pay $100 (U.S.) cash for a
simple office visit to an English doctor, plus cost of all prescriptions.
When we were with a group in GB, we stayed at a wonderful inn out in the country. After dinner I had to accompany two elderly ladies who had each taken a fall during the day. The hotel took us to the hospital where they were x-ray'd and pronounced well. When we got back several hours later I learned that the Inn was haunted and I had missed the haunted tour. Darn.
But, since as you say, it was a medical emergency, they weren't charged anything.
