If you mean to drive in the city proper, I would say a definete NO to both. George and I have driven in many parts of Italy and Greece and enjoyed every single Km.
However, Cairo seemed to us quite bewildering. Cars, buses, donkeys, oxen etc, all vie for space on the road and noone seemed to pay any attention to the very few traffic lights we saw. I would not enjoy sightseeing at all if I had to drive. I'd rather shoot myself in the foot.
Athens is another matter. Like any large cosmopolitan city, it is very overcrowded with cars. They limit the cars in the city by the tag (license plates) odds and even depending on the day I believe. (Joanna would know best here). But, if you can avoid 'rush hour' and you dont mind the crush....go for it.
Rome, with the mopeds and motorcycles would be the third city I would not drive around either. Cant enjoy what I'm doing if Im that tense.
On the other hand, renting the car and leaving Cairo or Athens for other parts of the country is a different thing altogether. Hope this helps!
