What can be really bad is when people from outside the US hire a car in the US, say with FlyDrive. They often haven't a clue about insurance, and either drive around drastically underinsured or find that they have a hefty insurance fee to pay. I found once that it was cheaper to pass on the FlyDrive option and to arrange my own hire. Having said that, the main problem is when you try to book direct. Travel Agents, at least in the UK, can get you some excellent buys on car hire in the US. But insurance is always a killer, as it isn't covered by our credit cards, etc.
