Your cousin will definitely need a shawl or a sweater or something. The Church Police are quite vigilant about shoulders and upper arms-- I saw a number of women be refused entry to churches on the basis of shoulders or thighs being exposed. I wore sleeveless tops almost every day in Italy, but kept a sweater tied around my waist or stuffed in our little pack (frumpier but more convenient to tie it around the waist, since it's easier to duck into a church on a whim), and it worked out fine.

I was, however, usually the only woman in any given church who was wearing a hat; covering your head, apparently, is no longer necessary.
