As a number of people have said, Siena is wonderful, but I don't feel it is a small town. Smaller than Florence, but not a little hill town. San Gimignano in Tuscany is wonderful, as mentioned. In Umbria the previously mentioned Orvieto is great and has the benefit of being close to Rome. In Umbria we love Spello, and the town of Spoleto is on a direct train route I think (we are drivers, so you should check).
