The FAQ for rec.travel.europe says that you should expect to pay about 1% more than the Interbank rate for a bank machine or credit card transaction. You would pay about 2 or 3% more for buying cash at a bank.

By the way, we found that traveller's cheques were useless - we had to go to the American Express office in Paris to change them. The bank wouldn't do it and the Bureau de Change wanted to charge a premium as if they were in foreign currency. They were euro traveller cheques!
